How To Refer Us A Patient

Please have your office fax or email ( Support@PAI.com ) the following information over to our office:

  • Patient Referral Information
  • Script
  • Demographics (include contact number)

  • Soap Notes

  • LABS

Please attach any prior diagnostic images and reports already done that are relevant to this case.

Our Patient care Coordinator will call and schedule your client. At the same time, our Board-Certified Vet will review the case, prep the correct path for anesthesia, and then call you if there are any additional questions.
